Complete this permission form if your student is to receive medication while at camp

The medication policy remains the same whether your student is at school or at camp. Please review this policy and complete the form if needed.

Listed below are a few reminders that will help you to prepare to send your student to Camp KOK.

1. Medications that are kept at school for your student are not sent to camp. Only those medications that you supply specifically for camp will be sent.
2. Your student may receive Tylenol while at camp only if you have given prior permission at the bottom of the Emergency forms at the beginning of the school year.
3. Your student's emergency form will be kept at the camp health office in case of injury or illness. Please make sure your contact information is accurate in Tyler Student 360.
4. Medications should be picked up by a parent/guardian when your student returns from camp. This is in accordance with the school health policy.
