Department/IMPACT TEST, PHYSICAL INFO REQUIRED FOR STUDENT-ATHLETES
TRYING OUT FOR WINTER SPORTS
All student-athletes in grades 7, 9 and 11 who did not
participate in a fall sport are required to take the IMPACT test prior
to tryouts on Friday, November 20th. Also required is a
valid CIPPE physical form turned in to the Athletic Office before that
date. Any student in 7th, 9th and 11th
grade must have both an IMPACT Concussion Test on file for this school
year AND a valid CIPPE physical form before they will be eligible to try
out for a winter sport. Any student in 8th, 10th
and 12th grade must have a physical form turned into the
Athletic Office before trying out AND must have had an IMPACT concussion
test taken some time in the 2008-2009 school year. If they did not take
an IMPACT test last year, they must take one this year.
If athletes don’t have a physical and IMPACT test on file before
November 20th, they will NOT try out.
They can fix the physical part pretty quick, but will NOT be able to fix
the IMPACT part after next week. With regard to
the physical form, if athletes played a fall sport and did not have an
injury, they just need to turn in Section 5 to the Athletic Office. If
they had an injury, they need to go back to their doctor and fill out
Section 4 again with an updated date.
