| IMMUNIZATION
REGULATIONS In
accordance with the Allegheny County Health Department, the immunization
guidelines for students are listed below. In order to
enroll, a student must have at least one dose of each required vaccine
listed below
You will have a provisional period
of eight months to complete the necessary requirements. If not
complete, your child will be excluded on May 1st of that school year.

Acceptable
documentation includes either:

The child’s immunization record from the Primary Card Provider.

A written statement from the former school district or from a medical
office that the required immunizations have been administered, or that a
required series is in progress.

Verbal assurances from the former school district or a medical office
that the required immunizations have been completed, with records to
follow.

Exemptions to
Immunization Laws
Medical exemption
- when the physical condition of the child is such that immunization would
endanger life or health. (requires Physician signature)
Religious
exemption –when the
parent/guardian adheres to a religious belief whose teachings are opposed to
such immunizations. (requires parent/guardian signature) |
