Just a reminder that Physical and Dental forms need to be completed and returned to your child's school as soon as possible. Both physical and dental exams can be completed from one year prior to the required exam year.  For instance, if a student starts 3rd grade on August 27, 2007, the third grade dental can be dated anytime from August 27, 2006.   We are unable to accept exams dated earlier. 

Physical exams are required for students in grades kindergarten, six, and eleven.

                            

Dental exams are required for students in grades kindergarten, three, and seven.
