Emergency Forms

The 2009-2010 Emergency Medical Authorization forms were mailed home for NAI and NASH or will be sent home with students on the first day.  These forms are to be returned as soon as possible to avoid any delay in our reaching you in case of emergency.  Please be sure to sign the form to verify your review of the provided information. 
 

 Medication Drop Off

Please be sure to drop off your child's medication with completed permission form and in the original container.

Just a reminder that Physical and Dental forms need to be completed and returned to your child's school as soon as possible. Both physical and dental exams can be completed from one year prior to the required exam year.  For instance, if a student starts 3rd grade on August 27, 2009, the third grade dental can be dated anytime from August 27, 2008.   We are unable to accept exams dated earlier. 

Physical exams are required for students in grades kindergarten, six, and eleven.
