These appraisal forms are to be given to three professionals who have observed you in the classroom or who can attest to your teaching performance. Act 34 Criminal Record Check; FBI Fingerprint Check [Effective April 1, 2007--Everyone must apply for an FBI Fingerprint Check by calling 1-888-439-2486 Monday through Friday 8 a.m. - 6 p.m. EST or via http://www.pa.cogentid.com using a credit card or debit card to register for an appointment.  Money orders or cashiers checks payable to Cogent Systems will be accepted on site for those applicants who do not have the means to pay electronically.];  and Child Abuse Act 151 History Clearance must be filed with PA-Educator along with your application.
